MS BMED Program Guides and Forms

Many forms require you to create a DocuSign account. You can visit eSignature.gatech.edu to create this account before initiating forms. 

Institute Graduate Student Guide: View Guide 

 

Thesis Related Guide and Forms: 

  • Institute Thesis and Dissertation Manual: View Guide 
  • Advisor-Graduate Student Thesis Commitment Form  initiated by student. Required once via DocuSign: Initiate form 
  • Advisor Financial Arrangement Form initiated by the advisor in the event of a financial arrangement.  Required every semester via DocuSign: Initiate Form 
  • Program of Study Thesis Form  initiated by student and signed on by Thesis Advisor and Masters Program Manager via DocuSign: Initiate Form 
  • MS BMED Thesis Defense Announcement Form initiated by student at least 14 days before the presentation: Initiate Form 

 

Non-Thesis Related Form: 

  • Program of Study Non-Thesis Form  initiated by student and signed on by Student and Masters Program Manager via DocuSign: Initiate Form  
  • MS BMED Project Permit Form initiated by student after discussion with Project advisor: Initiate Form 
     
 

Other Forms:  

  • Enrollment Waiver Form: use this form to request a waiver of the Institute requirement for registration during the semester of graduation:  Initiate Form 
  • Graduate Change of Major Form: Use this form if you are changing your major from the BME department and into another program outside of BME via DocuSign:  Initiate Form
